May be due to chondromalacia patella. In this condition the cartilage on the underside of the patella softens and deteriorates. Some people are able to ignore the condition, but it will not improve and will probably need surgery. Sometimes an unstable flap of surface cartilage may cause this pain, and may be curable with simple keyhole surgery and a chondroplasty.
Severe Knee Pain & Instability
Severe knee pain is often accompanied by instability. There may be two things going on. Firstly, if the knee gives out completely i.e. collapses when you have weight on it so that you stumble or fall, it is most likely that you have ruptured one of the knee ligaments, such as the ACL.
The ligaments are the main stabilising structures in the knee, so if they rupture the knee will often give way, particularly when you are twisting or turning. Ligament ruptures typically occur during twisting injuries and are often accompanied by an audible pop or cracking noise.
The second possibility is that the body is trying to protect itself from further damage. When you take weight on the injured leg the pain can be so severe that it feels like the knee is going to buckle or give out, but it doesnt actually collapse. This usually means there is damage inside the joint, but it is unlikely to be ligament damage.

What Causes A Swollen Knee
- Injury a meniscus tear, torn ligaments , overuse, blunt force from a fall, or a fracture/break.
- Osteoarthritis as cartilage wears down, joint fluid can get overproduced and cause swelling. Often, this leads to discomfort and pain. Other types of arthritis which can cause swelling in the knee in rheumatoid, septic, and reactive arthritis.
- Bursitis the bursae are fluid-filled sacs in the knee that help to provide cushion and decrease friction between bone and soft tissue. These bursae can become inflamed causing them to fill with excess fluid.
- Gout this is a condition which is caused by an accumulation of uric acid within the joint. Swelling often occurs at night while lying in bed. Gout most often occurs in the big toe, but can also present in the knee, heel, ankle, and foot.
- Pseudogout- this is a condition which is caused by an accumulation of calcium pryophosphate crystals within the joint. This condition is most common in the knee, but can present in the shoulder, ankle, wrist, fingers, spine, or hips.
- Osgood Schlatter describes a condition in which the patellar tendon of the knee becomes inflamed. This condition is most common in active adolescents and young adults in their 20s.
- Bakers Cyst- this occurs when a bursa in the back of the knee fills with excess fluid.
- Obesity, age, sports activities, and overuse can contribute to or cause a swollen knee.

The Best Supplements For Knee Pain

Swelling Behind The Knee

There are a number of different causes of swelling behind the knee.
The most common is a Bakers Cyst where there is inflammation of the popliteal bursa at the back of the knee.
Sometimes there is back of knee swelling and pain, other times there is a lump behind the knee but no pain associated with it. It might be that only one knee is swollen, or there may be swelling behind both knees.
Most times when the back of the knee is swollen, it can be treated with a combination of rest, regular ice, compression bandages, exercises and physical therapy, but some case may require knee surgery.

Rare Causes Of A Swollen Knee
1. Knee Cap Dislocation: The patella usually glides in a groove at the front of the knee but a forceful injury can push it out to the side, resulting in a misshapened, swollen knee.
2. Patellar Tendonitis:Irritation of the patellar tendon can lead to mild swelling at the front of the knee.
3. Tumour:There are various types of tumour that can cause knee swelling. They are often accompanied by fatigue, weight loss and a general feeling of being unwell.
4. DVT: Deep vein thrombosis is a blood clot in one of the deep veins, most common in the calf or thigh. They are normally painful, hot and red and are most common after prolonged bed rest, surgery or air travel. A DVT is a medical emergency – if you suspect you may have one see your doctor immediately.
5. Spontaneous Haemarthrosis: Sudden bleeding into the joint in someone with a blood clotting problem or who is taking blood thinners e.g. warfarin.

What Causes Pain Behind The Knee When Bending
The most common cause of pain behind the knee when bending is a Bakers Cyst. This is when there is inflammation of the semimembranosus bursa, a small sac filled with fluid that sits at the back of the knee.
If the bursa gets inflamed, then any time you bend your knee, the bursa gets squashed, causing posterior knee pain.
How Common Is Gout In The Knee
As a general rule of thumb, if left untreated, gout tends to work its way up the body, Dr. Keenan explains.
For example, he cites research that shows 50 percent of patients experience their first gout attack in the big toe. If gout worsens, 35 percent of secondary flares occur in the knee, 40 percent in the midfoot and ankle, 30 percent in elbows and wrists, and 15 percent in fingers.
Its not uncommon for a person to experience their first gout flare in their knee and, after an X-ray or ultrasound, show signs of gout in the foot, he adds.
Gout can afflict both knees, but typically is felt more strongly in one knee where arthritis from general wear is worse.

How A Swollen Knee Affects Your Range Of Motion
Range of motion can be affected especially when swelling occurs in or near the joint. Increased swelling can also cause pain which limits the individuals range of motion.
Swelling is known to cause inhibition of both motor and sensory neurons. This means that muscles lose the ability to fully fire leading to problems with movement.
It also leads to decreased proprioception, or a persons ability to sense where they are in space leading to balance deficits and potential falls.
In addition to decreased range of motion, the skin around the knee swells and becomes puffy. Weve seen instances where swelling causes pants to fight tightly and, in some cases, not fitting at all.
