I had knee replacement surgery 6 weeks ago . Through PT I have been working on breaking up the scar tissue only for it to regrow by the time I get back to PT two days later. I have been massaging at home, using a hand held massager and roller. It is painful and swollen. I am getting very disheartened. Any suggestions as to what else I can do. Has anyone had laser treatments to break up scar tissue? Were they effective?
Hi there . yes, Ive had scar tissue develop also. It developed slowly over time, but by the time I hit the 1 year mark, my surgeon went in arthroscopically and cleaned it up. But, it never was right, and in another year I had to have a revision a totally new replacement. Since then, my knee could not be any better. I dont know why this happens to some and not to others but after several other options had been tried, including the arthroscopic surgery, I opted to just do the revision . Im glad I did.abby
hi relating but early in my recovery, 5 months, my MD says if scar tissue does not go away I will get arthroscopic procedure. My knee was only damaged in the knee cap, the other sides are good. When you had your knee replaced were the other parts of the knee damaged and in need of replacement? Or was it a decision to go to TKR suggested to solve the problem because partials dont really work?

Flexion And Extension Contracture
Contracture is a shortening of the muscle itself. So, fibrosis is something that sets up, like we talked about, as a spiderweb, and it connects bone to bone. Contracture is an actual shortening of the muscle, the tendon, because its been in a short position for so long. You know that whole use it or lose it? Thats real. That really happens. And so if you dont move that muscle, its going to contract, its going to stay short, because thats all it ever had to do. And thats why sometimes strengthening after a knee replacement takes longer, because you most likely didnt have that range of motion before surgery.
If you had a knee, and that knee was only bending to 70 degrees, then the quadriceps knew that it had to work from 70 to zero, and thats all the quadriceps had to do. And the hamstrings only pulled the knee back to 70 degrees. They didnt pull it back any further, right? So, those muscle fibers that exist after 70 degrees on either side of the joint, theyre on vacation, and they like being on vacation.
And when you ask them to start working again it takes a little bit of time for them to remember what their goal was, what their target was, what their job is.

The X10 Knee Recovery System
I think the X10 helps prevent scar tissue in general. ;If we can prevent general scar tissue, we certainly can prevent scar tissue on steroids, when you talk about something like a fibrosis, because you get in and you get moving early. Weve had physicians who love the progress of their patients so much, they dont want to put the CPM in the hospital, they want to bring X10 into the hospital, okay? And thats amazing, to take someone whos an orthopedic surgeon whos been an orthopedic surgeon for a long time, whos relied on the CPM for a long time and have them say, I dont want to do that anymore. I like the X10. I like how it works so much, Im going do it even sooner. I want that patient on the X10 the hour after they come out of surgery or in those kinds of parameters.
And so X10 as far as preventing fibrosis I think is absolutely what it does. But beyond that, I think if you have a patient that has fibrosis in their knee, or has some kind of a contracture, you can also bring X10 in in that situation, because of the ability to alter the hold times on the machine. On the X10 you can hold in a challenging but not painful position for three seconds, or for longer than that. With the fibrotic joint, I just need to hold longer. So even if a joint is fibrotic, we may not have to worry about a manipulation.
When The Scar Heals
- Push & Pull:;Imagine that you have divided the scar into sections 2 finger widths apart. Place 2 fingers in the left most section and move it in all directions of a clock . Where ever the scar feels stuck hold this position for 60 seconds. You will feel a strong pulling or light burning. Then move to the next section until you have massaged the whole length of the scar. When the top layers of skin move more freely try to press a little deeper to get to a deeper layer of the scar.
- Skin Rolling: Pinch the skin on either side of the scar, lifting the skin up. Start at either end and move forward and backward, rolling and raising the skin as you move. A free scar bulges upward, a stuck scar will dimple.
- Stroking along the Scar:Massage the scar tissue by working it with a rubbing motion along the grain. You can move your fingers apart along the length of the scar as if you are trying to make the scar longer.
- Plucking: Put your index finger on one side and thumb on the other side of the scar. Try and lift scar up, separating it from the underlying tissues. If the skin slips out of your hands, you may not be ready for this stage.
The sensation you feel when you massage your scar is one of strong pulling or light burning, NOT sharp pain. Sharp pain is as if you cut your fingers with a knife. Scar massage should NOT feel like that.

TKR causes some serious trauma to your knee joint.
Your surgeon will make a large incision directly over the top of your knee. Once the surgeon has you opened up, the bone is either cut or shaved .
Small holes are drilled in your bones. Your muscles and supporting structures probably have atrophied before surgery .
In my case my knee had bowed and the surgeon physically straightened it. During the process, my ligaments and tendons were stretched in ways they hadnt been in decades .
Often times, surgeons remove your ACL too. My ACL was missing from previous injuries and my surgeon jokingly thanked me for not having to remove it during my TKR .
After surgery you have to deal with the wound , also with the drainage , swelling and the stiffness that results. The physical therapy starts hours after surgery; the hospital physical therapist had me up next to my bed and doing my initial range of motion exercises .
this point, I was medicated and still had the drainage tube attached to my knee.
