Update To Coding Degenerative Changes

Guest blog by Monita Turpin, coding analyst at 3M Health Information Systems.

In my last blog, I wrote about the conundrum of assigning ICD-10-CM code when providers only document degenerative changes.

I wrote my blog in early June 2018. The day before my blog was published, I received notification of an updated Coding Clinic, Second Quarter ICD-10 2018 Page: 15 Effective with discharges: .

So, lets revisit the scenario with the clarification from Coding Clinic:

The provider documented degenerative changes of the left knee in the impression, and the clinical indication was left knee pain. What is the correct diagnosis code?

The question that was submitted to Coding Clinic was:

What is the correct code assignment for advanced degenerative changes of both hips when documented in the final impression section of an outpatient radiology report?

According to Coding Clinic: Assign code M16.0Bilateral primary osteoarthritis of hip for degenerative changes of hips.

Coding Clinics rationale is, ICD-10- CMs Alphabetic Index under Degeneration, joint disease instructs see Osteoarthritis.

This is a major change that some will say has not completely colored in the grey area for degenerative changes because the provider did not specify joint disease however, we must code based upon guidelines and when guidelines are not clear, we must go to Coding Clinic for guidance.

The Controversy Surrounding Hollywoods Home Health Checks

Bilateral Total Knee Replacement with Mikey Mike 12 days Post Operation TKR Sports Quick Recovery

The controversy surrounding Hollywoods home health checks started when the Associated Press reported on June 16 that the Academy Awards were to feature a segment on home healthchecks. However, after outcry from those who felt that promoting home healthchecks was tantamount to promoting elder abuse, the Academy pulled the segment from their broadcast. In response, ABC News released a statement claiming that they had been unaware of any controversy surrounding Hollywoods use of home health checks and that they would continue to use them in future broadcasts.

Since then, more information has come to light about Hollywoods use of home health checks. As previously mentioned, the Academy pulled the segment after outcry from those who felt that promoting home healthchecks was tantamount to promoting elder abuse. However, evidence has since emerged which suggests that this was not actually the case. For example, a study published in BMC Public Health found no link between regular visits by family members or caregivers and any form of abuse or neglect among seniors living at home.

Another issue which has come to light is Hollywoods reliance on third-party companies to provide their home healthcheck services. In some cases, these companies have been found to be unreliable and have engaged in unethical practices such as fraudulently billing Medicare for unnecessary services. This has led many people to question whether or not Hollywood should continue using these services at all.

How Is Bilateral Knee Oa Diagnosed

ICD-10 Coding Clinic Update (Q1 2017): TKR and Patellar Component

Bilateral knee OA is diagnosed through a combination of physical exams and tests. At your first visit, your doctor will check for redness and swelling in your knees. Your doctor will also ask you about your history of knee pain and any past injuries.

Possible tests include:

  • X-rays to help provide detailed images of the knee bones and rule out other forms of arthritis
  • blood tests to rule out rheumatoid arthritis

Imaging tests are performed on both knees if bilateral knee OA is suspected. Your doctor will also note any differences in how OA has affected one knee versus the other. These details could help influence treatment measures and possible surgery recommendations.
