If you have rheumatoid arthritis, you have an unhealthy immune system that triggers inflammation in your joints even though theres no foreign invader. The inflammation causes pain, stiffness and swelling of the synovial membrane, which can also wear away your cartilage.

- CT scan of the knee
- Joint fluid culture
Your provider may inject a steroid into your knee to reduce pain and inflammation.

Your Doctor May Not Want To Give You Cortisone If Knee Replacement Is Seen As Ultimately Your Only Answer
We see many patients who tell us that they have gone to their doctors and have asked for one more cortisone injection because of the amount of pain and swelling that they were suffering from that day. You know that the call to the doctors office for an appointment usually comes on the day when your knee hurts worse. The doctor, to his/her credit in many cases, has declined to give his/her patient this one more cortisone shot because their concern is that if you get cortisone injections into your knee prior to surgery, you will have a greater risk of complications after the surgery. There is a lot of debate around this subject.
A December 2020 study published in the medical journal Rheumatology gives this overview assessment of the debate surrounding the use of cortisone for a bone on bone knee. Here are the summary learning points:
- Existing data indicate that intra-articular corticosteroids in knee osteoarthritis provide short-term pain relief and functional improvement which may last from one to several weeks.

What Really Solves Knee Pain The Answer Might Surprise You
The pill most often prescribed for knee pain acetaminophen may not help at all, researchers reported Monday.
The new study of what works for arthritis pain in the knee came up with some surprising results. The most common treatments often may not help much, and even a placebo injection of salt water provides more pain relief than any pill.
The findings go against what many doctors have long believed, although they didnt surprise veteran knee surgeons, who know pain relief varies greatly from one patient to another.
All treatments except acetaminophen showed clinically significant improvement from baseline pain.”
Dr. Raveendhara Bannuru and colleagues at Tufts Medical Center near Boston looked at thousands of studies on various treatments for knee pain, from acetaminophen the active ingredient in Tylenol to ibuprofen to steroid injections.
Finding the scientific truths is tricky. Theres not much incentive for drug companies to test generic, over-the-counter drugs such as naproxen, and theres also not much reason to test an injection of a steroid against a generic treatment such as hyaluronic acid, a kind of joint lubricant.
But the team came up with 137 studies covering 33,000 people that met some high standards theyd been blinded, meaning the doctors didnt know which treatment a patient actually got as they evaluated pain, stiffness and range of motion.
And most compared treatment to placebo a dummy pill or dummy injection.

When a patient reports to the doctor that they have problems with swelling, the doctor should consider the problem worse than the patient is suggesting.

- Synovitis is a prevalent feature in patients with knee osteoarthritis and meniscal tear and is associated with pain and cartilage damage.
- The researchers analyzed data from 276 patients. The patients self-reported their swelling episodes.
- Twenty-five percent of patients reported no swelling,
- 40% of patients reported having intermittent swelling,
- and 36% of patients reported having constant swelling.
When these patients had an MRI. The MRI found much more swelling than the patients reported. The conclusion of this study urged doctors to use caution against using patient-reported swelling as a proxy of inflammation manifesting as effusion-synovitis. In other words, the swelling is worse than the patients think it is. Simply, the knee is worse than they think it is.

The 4-step R.I.C.E. Formula
1.Rest will give the joint time to repair and recover. Take a break from sports and other activities for 24 hours or longer. The joint should not necessarily be immobile people with knee swelling should try to gently flex and straighten the knee several times a day to maintain range of motion.
2.Icing is an easy, effective treatment for excess knee swelling. Cold therapy can ease symptoms by:
- Causing nearby blood vessels to constrict, decreasing blood flow and inflammation
- Slowing down the production of joint fluid in the knee
- Distracting the brain from pain signals to the brain
Apply a cold compress to the knee for no longer than 20 minutes at a time. This can be done several times a day.
Ice should not be applied directly to the skin. Prevent skin damage by placing a towel or another material in between the icepack and skin. Cold therapy may not be appropriate for people who have Raynauds Syndrome or nerve damage.
3.Compression involves wrapping the affected joint in an elastic bandage . Compression may help limit or reduce swelling.
