What Is Prp Therapy For The Knee
Knee arthritis is the most common condition I encounter with patients who inquire about PRP therapy for the knee.
Knee arthritis is where the coating on the ends of the bones called cartilage starts to wear.
There can be a broad spectrum relative to knee arthritis:
- Mild to moderate arthritis
- Severe arthritis
Patients with mild and moderate arthritis typically have enough cartilage left where invasive surgical procedures, like knee replacement, are not appropriate for them, but theyre still having symptoms of arthritis. Those are the patients where PRP therapy for the knee can be very effective in improving both pain and function and potentially delay or prevent the need for knee replacement.
Patients with more severe arthritis may benefit from PRP by delaying the need for knee replacement for several years, although knee replacement may ultimately still become necessary.
In addition to knee arthritis, PRP therapy for the knee may also be beneficial in conjunction with surgical procedures such as ACL procedures or meniscus procedures. PRP may be added to augment the healing response and possibly reduce failure rates.

We will often get emails from people who had previous PRP therapy without the desired healing effects. We explain to these people that their treatment probably did not work because the single PRP injection did not resolve knee instability. The PRP may have tried to create a patch in the meniscus or cartilage to help with a bone-on-bone situation but the instability and the wear and tear grinding down the meniscus and cartilage remain.
When a person has a ligament injury or instability, the knee becomes hypermobile causing degenerative wear and tear on the meniscus and knee cartilage. In other words, the cells of the meniscus and cartilage are being crushed to death. When you inject PRP cells into the knee, without addressing the knee instability, the injected PRP cells will also be subjected to the crushing hypermobile action of the knee. The single injection PRP treatment will not work for many people. The knee instability needs to be addressed with comprehensive Prolotherapy around the joint. Prolotherapy is the companion injection of simple dextrose. This is explained in detail below.

More recently a June 2021 study from a team of European orthopedic surgeons led by the Orthopedic Arthroscopic Surgery International Bioresearch Foundation discussed all the research which points to all the factors that could make PRP a successful treatment and those which could jeopardize PRPs effectiveness. The Evidence For Prp Knee Osteoarthritis Treatments As An Effective Treatment
The basics behind how PRP works for knee osteoarthritis are summarized in research from doctors at the University of California. In a study in the publication of Tissue Engineering. Part B, Reviews, the doctors suggest that PRP injections cause positive, beneficial, and healing cellular changes in the joint environment. These changes help move the knee from degenerative knee disease to a more healing and regenerating knee joint environment. Healing includes: regeneration of articular cartilage, increasing the volume of natural knee lubricants, and waking up the stem cells present in the knee to assist in the transformation to a healing environment.
In the present study, the researchers wrote: PRP modulates the repair and regeneration of damaged articular cartilage in the joints and delays the degeneration of cartilage by stimulation of mesenchymal stem cell migration, proliferation, and differentiation into articular chondrocytes .
- What this last sentence means is that stem cells in the knees, cells that are responsible for repair on many levels,
- migrate to the area of degeneration because PRP called them to the site of the injury,
- proliferate the stem cells made more of themselves,
- differentiate the stem cells changed themselves into cartilage cells
Prp Treatment For Knee Side Effects
PRP injections are a relatively safe procedure that usually only have minor side effects. Due to the need for a blood draw, you should drink plenty of water and eat well ahead of time to avoid feeling lightheaded. After the injection procedure, you may experience some discomfort and bruising at the injection site.
Platelet Rich Plasma Injections are used to treat a knee injury. There is only a slim chance of side effects. When PRP injections for knee discomfort are performed by unrelated factors, there may be side effects. Because of the expert therapistâs assistance, the treatment will proceed as planned. Do I Need To Prepare For A Platelet
Usually, we ask you to be well hydrated. We suggest you drink 1 to 1.5 litres of water on the day of the PRP injeciton.
Also, you must avoid medications that affect platelets, such as aspirin or ibuprofen, for at least ten days before the injection. In addition, you should avoid these same medications for a few weeks after the last injection.
The Possibility Of Regenerating Cartilage
There is no doubt that cartilage regeneration can be done. Before placing the implanted cells in the damaged area, chondrocytes, which are the cells that make cartilage, can be extracted from the patient and cultured in a laboratory. This procedure is known as cartilage implantation procedure , and it has the potential to restore damaged knees.

How Many Prp Injections Are Needed For The Knee
The number of PRP injections needed for the knee can vary depending on the condition being treated.
There are tendinopathies or issues with the tendon, like quadriceps tendon and patellar tendon, which tend to be more like a single injection or one additional injection.
When it comes to using PRP for knee arthritis, all the published clinical literature has looked at the efficacy with the use of a series of three injections.
I personally mirror my practice after the available literature. I expect patients to have the same results seen in the literature. So as an example, for knee arthritis, I tend to recommend a series of injections done approximately a week apart.
Tendon Ligament Muscle And Joint Injuries
PRP injections may be able to treat a range of musculoskeletal injuries and conditions. Prp Protects Against Cartilage Loss
A November 2019 study examined the outcomes of PRP administration in the osteoarthritic knees of laboratory rats and concluded that PRP had induced an inhibitory effect on the apoptosis of chondrocytes , which, in turn, prevented the loss of cartilage height by inhibiting matrix loss.
A January 2020 study found similar results in humans and took before an after MRIs to document these findings. In this study doctors examined patients with bilateral knees osteoarthritis-grade 1, 2, and 3 Each patients knees were randomly allocated to either control or treatment groups. PRP was injected in two sessions with 4 week intervals in PRP group.
46 knees were included in this study. 23 knees in the case group and 23 knees in control group were studied.
All patients were female with mean age of 58. An MRI was taken before treatment and an MRI was taken after treatment.
In the PRP group, all of the radiologic variables , with the exception of subarticular bone marrow abnormality, had significant improvement. In a comparison between the two groups, patellofemoral cartilage volume and synovitis had significantly changed in the PRP group.
What Is Prp And How Is It Collected
PRP is the straw-coloured plasma component extracted from your own whole blood, containing a rich concentration of platelet cells with their associated anti-inflammatory and growth factor proteins in amounts up to 2 times greater than usually found in whole blood. Once 15mls of your blood is collected , a centrifuge is used to separate the red cells to the bottom of the enclosed syringe, leaving the plasma containing the concentrated platelets to be extracted from the top. The PRP is then injected by your doctor into your knee joint or tendon whilst it is fresh.

Clinical Trial Denounces Use Of Platelet
Injections of platelet-rich plasma are no better than placebo for helping reduce knee pain in people with knee joint osteoarthritis, an Australian clinical trial led by researchers at the University of Melbourne, University of Sydney and Monash University has found.
Knee joint arthritis affects more than two million Australians with platelet-rich plasma injections becoming a popular form of treatment for painful joints in recent years. Platelet-rich plasma uses a concentration of a patients own platelets injected directly into the joint with the aim being to aid in long-term pain relief and to improve joint cartilage.
The research, published in one of the worlds leading medical journals, the Journal of the American Medical Association, compared a series of three-weekly injections of platelet-rich plasma to three-weekly injections of saline, which acted as a placebo for the study.
Participants and injecting doctors were not aware of whether there was platelet-rich plasma or saline in the syringe during the injections. Participants were tracked for pain levels and cartilage health over 12 months.
Researchers found that while participants who received platelet-rich plasma injections had a significant improvement in their knee pain over 12 months, the level of improvement was matched by those in the placebo group. There were no differences identified in the MRI scans across both groups.
