    Large nerves exit and enter the vertebral column to transmit information to and from the extremities and brain about sensation such as sharp and dull touch, and to control movement, such as walking.

    These nerves are located very close to the discs and joints of the spine, so with enough stress to the surrounding structures, the space that contains the nerve becomes crowded and the nerve can become irritated and inflamed.

    Pain That Originates In The Front Of The Hip

    Hip pain that occurs in the front of the hip and groin area is usually caused by conditions that affect the hip joint.1 A few examples are discussed below.

    Hip osteoarthritis

    Wear-and-tear of the hip joint, called hip osteoarthritis, commonly causes deep aching pain in the hip and groin region.1,2 The pain may spread to the front of the thigh and knee, sometimes including areas below the knee.2 The pain is usually worse in the morning, after prolonged sitting or resting, and/or physical activity. A locking, sticking, or grinding sound may occur during hip movements.3

    Hip labral tear

    When the labrum or the cartilaginous ring around the hip socket tears, the symptoms can be variable. Most often, labral tears cause pain in the groin. Pain can also occur along the side of the hip or buttocks.

    Initially, the pain from a labral tear may be felt during or after exercise and other vigorous activities. Over time, pain may also be felt during less strenuous activities, such as sitting.

    Hip osteoarthritis and/or labral tears may result from abnormal contact between the hip jointâs bones, resulting in hip impingement. Hip impingement pain may travel down from the front and side of the hip to the front of the thigh and knee. Sitting, driving, squatting, or performing hip movements and rotations typically aggravates this pain.4

    Conditions With Similar Symptoms

    A number of conditions that are not actually related to the hip joint can cause hip joint pain and symptoms in the “hip” area. These include:

    Spinal stenosis This condition most commonly causes pain in the buttock, low back, and back of the upper thigh . Spinal stenos is a lower-back problem, not a hip problem. Spinal stenosis causes pain in the buttock area that some identify as part of the “hip.”

    Greater trochanteric bursitisThis causes pain over the point of the hip . It also causes tenderness and sensitivity to pressure. Although this seems like a hip problem, it is a problem well away from the joint itself and is related to an inflammation in a lubrication point called a bursa. Greater trochanteric bersitis is not a joint problem .

    Non-orthopedic conditionsVery occasionally, non-orthopedic conditions can cause pain in the groin that masquerades as hip joint symptoms ovarian cysts, hernias, and other intra-pelvic conditions can sometimes cause pain that is mistaken for hip joint pain.

    Other types of arthritisOther forms of arthritis can cause similar symptoms to osteoarthritis of the hip in particular, post-traumatic arthritis and avascular necrosis are almost indistinguishable in many cases from osteoarthritis of the hip.

    The diagnosis of osteoarthritis versus rheumatoid arthritis can be made by a physician with experience in treating conditions of this type.

    Unable To Do Straight Leg Raise Knee Injury

    If you are unable to do a straight leg raise, it is likely that you have a knee injury. This could be a torn ligament, meniscus tear, or patellar tendonitis. All of these injuries can be very painful and make it difficult to walk or even stand. If you think you may have a knee injury, it is important to see a doctor or orthopedic specialist as soon as possible.

    A straight leg raise is a great way to increase the strength of your hip muscles and the muscles in your quadriceps. Shear forces, such as those that abnormally pull your shin forward or sideways, may interfere with your knee ligaments while they are healing after surgery. This disruption could cause your ligaments to re-injure and keep you from recovering. It is common for your surgeon to require you to wear a brace on your knee following ACL or lateral release surgery. In addition to protecting your knee from sideways forces, your brace aids in the prevention of knee joint instability. When using a brace, you raise your straight leg the same way you would if you were using a brace without it. You should increase the number of straight leg raises you perform when you can do two to three sets of the raises without weight.

    Older Patients With Arthritis

    Some patients, particularly middle-aged and elderly patients with hip and/or knee arthritis, experience a strange sensation thats called referred pain. Referred pain is essentially pain thats felt in a different place. This happens because the nerves that make us feel pain run through our body: the nerves that supply the hip also travel to the knee, and if the nerve is irritated at the hip, you may feel it at the knee.

    Osteoarthritis is the wear and tear of a joint. It means that the padding in the affected joint wears away. Surprisingly, the amount of young people with hip and knee osteoarthritis is growing, but we dont yet understand why. Recently, I performed a hip replacement on someone as young as 22 years old.
