Walking and cycling are good choices. But if land-based exercise is too painful, try an aquatic workout.The buoyancy of water is gentle on joints and may make it easier to work out. Aquatic exercise also provides a small but measurable decrease in pain and disability in people with knee and/or hip OA, according to another Cochrane review, published in 2016.
Ask about physical therapy. If pain and swelling make it very challenging to be active or youre unsure which exercises are best for you, see a physical therapist.

When your body is at rest, you may notice the mild to moderate pains that you could more easily ignore when your mind was preoccupied with busy thoughts during your waking hours.Additionally, when you sleep, your body produces less cortisol. Most of you who have all three of the findings I mentioned above will usually have very severe pain. For some of you, your Doctor will focus only on the meniscus and recommend surgery to remove the torn meniscus.

A better way to run is on a treadmill or track, or alternate jogging with walking, Dr. White suggests. A blood clot in the veins of your lower leg is called a deep vein thrombosis or DVT. You may have a blood clot behind your knee if you have one-sided leg swelling, pain, warmth, and redness below the knee. Sometimes these clots can occur on both sides at once, but this is uncommon. Does The Patient Require Medication
When nonpharmacological intervention proves unsatisfactory, multiple guidelines recommend acetaminophen as a first-line agent for mild to moderate arthritis. Because of reports of gastrointestinal adverse events, elevated hepatic enzymes and overdose, the OARSI guideline recommends conservative dosing and treatment duration of acetaminophen. Although the OARSI guideline does not give exact parameters on this recommendation, the AAOS guideline indicates it may be prudent to restrict the over-the-counter dose of acetaminophen to 3000 mg per day and reserve the 4000 mg per day dose for prescriptions.
Second-line agents include oral and topical non-steroidal anti-inflammatory drugs , selective cyclooxygenase-2 inhibitors and topical capsaicin. Use of oral and topical NSAIDs received a strong recommendation in the AAOS guideline.
For patients with osteoarthritis localized to one or both knees, the OARSI guideline recommends the use of local medications, including topical NSAIDs and corticosteroid injections . This was especially emphasized in patients with coexisting medical comorbidities, which are very common in this patient population. Each medication carries unique safety and adverse-effect profiles, and therapies should be specific to each patients individual risk factors and medical comorbidities.
Injections Are Another Low
If other strategies dont provide enough relief, injection therapy is an option with low risk.
A corticosteroid injection involves delivering this anti-inflammatory drug directly to the knee. The benefits are typically short lived. But it varies from person to person. I tell my patients the pain relief can last anywhere from a week to a year, says Dr. Day. One cautionary note with corticosteroids is the potential to increase blood sugar, which is a concern for people with uncontrolled diabetes.
For a possibly longer lasting effect, an injection of hyaluronic acid can be tried. Hyaluronic acid is a substance that healthy joints have a lot of and arthritic knees dont, says Dr. Day. It takes longer to start working than a corticosteroid injection, but the effect often lasts six months to a year.
Two other injection therapies are platelet-rich plasma and stem cells, neither of which is covered by insurance. PRP involves drawing some blood, spinning it in a centrifuge, and injecting part of it into the knee. Theres currently more science backing up the effect of PRP than stem cells, says Dr. Day.
If youre not able to get your symptoms under control with a combination of these measures, she says, it could be time to talk to your doctor about surgery.

Like other pain symptoms, some knee pain is not severe enough to require medical attention. You can manage the following symptoms at home:
1. Mild to moderate knee pain after a strenuous activity that you dont normally do, like running or walking a few miles.
2. Mild to moderate knee pain that comes on slowly.
3. Mild to moderate knee pain from minor injuries without significant movement limitation or inability to bear weight, such as mild knee strains.
