Due to their ages, many participants wanted to delay or avoid joint replacement. However, the recovery process after ACI is long, and respondents recognized that they would require 6 to 12 months of extensive rehabilitation to have the greatest chance for successful outcomes. Participants described the importance of attending therapy regularly and being committed to the process therapy provided the accountability participants needed to stay focused on the goal of continued progress, yet several acknowledged that, once they were discharged from therapy, they found it difficult to allocate the time needed to maintain and improve on the progress they had made during therapy. Amy recognized this when she said, Physical therapy was good because it made you do it. I mean, you were going in 2 or 3 times a week. So you were pretty much doing it. This procedure takes approximately an hour to complete and requires a 2-inch incision.

MACI is contraindicated in patients with a known history of hypersensitivity to gentamicin, other aminoglycosides, or products of porcine or bovine origin. MACI is also contraindicated for patients with severe osteoarthritis of the knee, inflammatory arthritis, inflammatory joint disease, or uncorrected congenital blood coagulation disorders. MACI is also not indicated for use in patients who have undergone prior knee surgery in the past 6 months, excluding surgery to procure a biopsy or a concomitant procedure to prepare the knee for a MACI implant.

Who Is A Candidate For Maci

UnitedHealthcares medical policy for MACI has now been expanded to cover adult patients with symptomatic full-thickness knee cartilage defects for the knee, such as a patella or multiple cartilage defects. The revised policy will go into effect on February 1, 2021.
Maci Surgery: A Highly Successful Treatment For Men

What Does Recovery Entail After Maci Knee Surgery
After the surgery, nurses will immobilize your knee. Immediate care after surgery includes the RICE method .
Doctors will prescribe medication to manage pain and discomfort. It is important to take these medications only as directed.
You will follow a physician-prescribed rehabilitation program that starts shortly after surgery. The goal is to decrease pain, improve mobility, and limit swelling and stiffness.
Also, follow any wound care instructions described post-operatively. Always report any signs of infection immediately.
You will be able to participate in light activities such as walking shortly after surgery. You may need assistance with ADLs for a short while or when using crutches.

It can be beneficial for people who have isolated cartilage damage in the knee, but it is not for those who have diffuse knee cartilage damage caused by arthritis. The articular cartilage and the nave are two types of cartilage in your knee joint.

How Surgeons Perform A Maci Procedure
Once the patient decides to undergo the transplantation surgery, the lab sends the cartilage to the hospital or surgical center where the surgeon performs the MACI procedure. The lab sends the cartilage in an aseptic state but not sterile. If something occurs and the patient can no longer undergo the procedure but the lab has already sent the cartilage, the surgeon must take a new biopsy and restart the process.
To start the cartilage transplant, the surgeon cuts the damaged portion of the cartilage and removes it. Using the same cutter and mallet used to remove the damaged cartilage, the surgeon then cuts into the new regenerated cartilage. Using the same tools allows the surgeon to create a piece of cartilage with the exact same shape and dimensions of the removed damaged cartilage. Using a sealant, the surgeon secures the new cartilage implant.
Autologous Chondrocyte Implantation Procedure
An autologous chondrocyte implantation procedure involves collecting articular cartilage cells from the patient and multiplying them in a laboratory to increase their number and then reimplanting with a special surgical technique to implant the patient
After implantation, you will need to go through a rehabilitation program. As a result of the bracing, it necessitates gradually returning to full weight bearing of the knee. Depending on the location of the lesions in your knee, you may begin weight bearing on the leg and gradually transition to a range of motion brace. Depending on the severity and location of the cartilage defect, the patient may require a knee brace for up to three months. To keep muscle muscles strong, static stretching and strengthening exercises are used in physiotherapy. It is not recommended to return to sports activities within twelve months of the procedure, and high-impact activities are avoided for two years.

What Is Maci Knee Surgery
MACI is innovative in that it involves regrowing the patient’s own cartilage cells in an FDA-licensed lab and then replanting them to repair the damage.
The technique is a two-step process. The first step is a knee arthroscopy, whereby the surgeon will look inside the knee to assess cartilage damage. The surgeon will note the extent and map the size of the cartilage deficit.
Surgeons will then take a cartilage sample to send to the lab, where the cells will be used to regrow more cartilage cells to be placed during the second procedure.
A 3D structure is created that mimics natural cartilage, allowing for healing and lowered risk of complications. The special cartilage membrane is then implanted into the knee, where it continues to heal and regenerate, repairing the cartilage deficit.
How Does The Surgeon Identify Candidates
First a surgeon will evaluate your knee with a physical exam. If there is concerns for a cartilage defect they will get an xray and MRI so they can better see all the cartilage surfaces. If you are missing a piece of cartilage you may be a candidate for MACI. Remember MRIs are only so good as these cartilage defects may be under sized on MRI. Currently, matrix autologous chondrocyte implantation is the preferred method of implantation in 3 generations of ACI techniques. Despite extensive nonoperative treatment, a 32-year-old woman remained in severe left knee pain. In the axial T2 MRI, a full-thickness cartilage defect along the lateral facet of the patella was detected, as well as small cystic changes in the subchondral region. An operation to correct this defect was recommended for this patient because she was over the age of 55, had a low activity level, and was of a defect type. During the procedure, the musculature of the anterior-lateral knee was lifted via a Cobb spinal elevator.
Who Is A Candidate For Cartilage Repair
Once bone-on-bone arthritis has developed, cartilage repair is not an option anymore, and patients can be considered for joint replacement surgery or osteotomy .
Since cartilage repair has a long recovery time of up to one year before the patient can return to unrestricted activities, such as running or basketball, it is mainly performed in younger, active patients where joint replacement is not an option. Even though we have treated suitable patients as old as 60 with cartilage transplants, this is rare and our average patient is between 16 and 45 years of age.
