Recent NICE guidelines on reducing risk of hospital-acquired venous thromboembolism recommend aspirin as an option, though not the switching strategy from rivaroxaban.

Recommended options after hip replacement are:

• Low molecular weight heparin for 10 days followed by aspirin for 28 days or
• LMWH for 28 days plus compression stockings until discharge or
• Rivaroxaban for five weeks .

• Results were similar across the different operations, and when looking at those already taking long-term aspirin or not.

Hello, this is Dr. Sam Goldhaber. Im president of NATF and Im speaking today on the Clot Chronicles on the topic of preventing pulmonary embolism and deep vein thrombosis in patients undergoing either total knee replacement or total hip replacement.

It turns out that the number one complication after total knee or a hip replacement has nothing to do with the orthopedic device thats inserted into the knee or hip. The number one complication is pulmonary embolism and DVT. How should we best prevent this?

Well, the guidelines are not very useful because they say you can do almost anything. Low dose aspirin, low molecular weight heparin, warfarin, or novel oral anticoagulants are all considered to be acceptable approaches to the prevention of venous thromboembolism after knee or hip surgery, specifically total knee replacement or total hip replacement.

So, this gives us another potential approach in our query and wish to stop DVT and pulmonary embolism after total knee replacement and total hip replacement.

The idea is to see which of these three regimens will have the lowest risk of venous thromboembolism.

Aspirin A Good Clot Buster After Knee Replacement

WEDNESDAY, Feb. 21, 2018 Good old aspirin is just as effective as newer, expensive drugs at preventing blood clots after hip or knee replacement, a new clinical trial suggests.

Researchers said the findings could change some doctors prescribing habits.

After knee or hip replacement surgery, theres a risk of blood clots in the legs or lungs. So its routine for patients to take clot-preventing drugs for some time afterward.

Right now, some doctors choose powerful anti-clotting drugs like dabigatran and rivaroxaban , said Dr. David Anderson, the lead researcher on the new trial.

But it hasnt been clear whether those expensive prescription drugs are any better than cheap, readily available aspirin, explained Anderson, of Dalhousie University, in Halifax, Canada.

Based on the new findings, theyre not.

Few patients in the study developed a blood clot after surgery, and those on aspirin fared just as well as those on rivaroxaban.

The caveat, Anderson said, was that all study patients received rivaroxaban for the first five days after surgery. After that, they either continued on the drug or switched to aspirin for another nine to 30 days.

From this study, we have no evidence to support starting aspirin on day one, Anderson said.

But after day five, he added, its very reasonable to consider switching to aspirin.
