Is Surgery Ever Needed For Anterior Knee Pain

In some cases of patella instability, where the kneecap dislocates often we need to consider surgery to reconstruct the ligament which holds the patella in place.

In cases of severe arthritis of the patella, we occasionally need to consider a joint replacement if you do not respond to a compression sleeve, injections, activity modification, and physical therapy.

Patella tendonitis or a jumpers knee will usually respond to activity modification and physical therapy. It is not unusual for your recovery to take up to 6-8 months. If the pain remains severe, there has been a recent interest in trying PRP injections to regenerate the patella tendon. It is still controversial whether or not PRP injections are useful. Surgery, although rarely needed, can be very useful in severe cases of a jumpers knee.

Other Inner Knee Pain Treatments

If your inner knee pain worsens after several days, or if basic at-home remedies dont alleviate symptoms, you should go see your doctor.

Some treatment methods for more serious knee injuries include:

  • Steroid injection. This injection is used to treat pes anserine bursitis.

Medial Collateral Ligament Injury

The medial collateral ligament runs along the outside of your inner knee to stabilize the joint. If the ligament overstretches, you may have an MCL sprain.

The MCL can also tear partially or fully. An MCL injury most commonly occurs after force is applied to the outer knee, such as in contact sports.

As we reviewed in a popular post about the sounds that our knees make, we discussed that the majority of these sounds are not mechanical or dangerous! That means that you are probably not bone on bone or grinding away the insides of your knee when you bend it. Yes, these sounds can be annoying or even embarrassing, but they are rarely dangerous. The most common cause of this grinding sensation or sound is actually due to inflammation of the tissues inside the knee. You do not need cortisone or lubricating injections to treat these sounds or sensations.

How Long Does A Knee Ligament Injury Take To Heal

This will depend on which knee ligament you have injured and also how severe your injury is. Also, if you have injured more than one ligament in your knee, recovery may take longer.

If you have surgery to repair your ACL, it usually takes around six months before your knee has recovered enough for you to return to your previous sporting activities. However, in some people, it may be longer. In general, surgery to reconstruct an ACL has good results in up to 8 out of 10 people in the short term but the risk of a second ACL injury is high, especially in young athletes. In the longer term, many people who have had an ACL tear develop osteoarthritis in their knee, whether or not they have had their ACL reconstructed.

If your PCL has been treated using surgery, it can take between 9 and 12 months before complete recovery.

After a stretch injury or partial tear to the MCL, the ligament has completely healed in most people after three months. If there is a complete tear, recovery may take a little longer but most people are back to their usual activities after 6-9 months.

Note: you should ask advice from your medical professional about when it is safe for you to start exercise or sport again after a knee ligament injury. It is important that you do not start exercising again until your knee is pain-free and completely stable. If you start exercising too early, you may cause further injury to your knee.

Medial Knee Ligament Sprain

An MCL sprain is a tear to the ligament on the inside of the knee joint. MCL stands for medial collateral ligament. It is caused either by a direct impact to the outside of the knee or from twisting. Symptoms include:

  • Sudden onset pain located on the inside of the knee.
  • Rapid swelling.
  • A positive result for valgus stress test.
  • Often they occur in conjunction with an anterior cruciate ligament tear. Or with a meniscus tear.

Conditions Associated With Pain At The Front Of The Knee

There are several medical conditions linked to pain at the front of the knee. They include the following.

Patellofemoral pain syndrome is one of the most common reasons for pain at the front of the knee. When you bend and straighten your leg, your kneecap slides up and down a groove at the end of your thigh bone . Patellofemoral pain syndrome describes pain related to this joint. It can happen if theres repeated stress on your knee for example, from jogging, squatting or jumping. It can also result from problems with the alignment between your kneecap and thigh bone. Doctors sometimes call it runners knee. It often happens in people who are very active particularly teenagers and young adults.

Patella tendinopathy means the patellar tendon in your knee becomes worn down. This tendon attaches your kneecap to your shin bone, and it can become inflamed and tear. This can be due to overuse and stress over time, particularly in sports that involve jumping like basketball and volleyball. Because of this, its often called jumpers knee. Quadriceps tendonitis is a similar condition but is less common. It causes pain and tenderness where the tendon from your thigh muscle attaches to your kneecap.

Infrapatellar fat pad syndrome is a condition where the fat pad below your kneecap gets pinched between your kneecap and thigh bone. Its usually due to repeated stress on your knee. Its sometimes called Hoffas syndrome.

How Is Chondromalacia Patella Diagnosed

A doctor will perform a physical examination of the knee to determine the cause of pain. If the diagnosis is not clear or symptoms do not improve a doctor may order one of the following:

  • Blood tests and/or a standard knee X-ray – This may help to rule out some types of arthritis or inflammation.
  • MRI scan- A tests that shows details of the knee joint and can reveal many cases of chondromalacia patella.
  • Arthroscopy – A tiny, flexible camera is inserted into the knee to see exactly what the cartilage looks like.

Treatment Of Pain At The Front Of The Knee

Whether or not you need further treatment for your knee pain and what treatment you have, will depend on whats causing the pain. Most conditions causing pain at the front of the knee can be treated with painkillers and physiotherapy. Surgery is usually only needed if other treatments havent worked.

Osteoarthritis is a common cause of knee pain and usually affects people over fifty years old. It occurs when cartilage which protects the bones degenerates. Symptoms include:

Pace yourself

If you are in pain dont tackle all your activities of daily living, such as housework, at once. Break the harder jobs down into smaller time frames and do something gentler in between. Sometimes a short rest when your knee starts to feel painful can be helpful over the course of a day. It is recommended to pace rather than to fully stop all forms of movement or exercise. You may find that taking simple painkillers as previously mentioned may help you to keep active.

High heels

Avoid high heels. This will alter the position of your leg and it can increase the strain into your knee and big toe.

Handrail

When climbing stairs you may need to consider using handrails and go up one leg at time until your muscles become stronger. It is better to go upstairs leading with the non-painful leg one step at a time. If coming down the stairs then lead with the sore leg one step at a time and use the handrail.

When Will My Kneecap Start To Feel Better

One broad caveat to keep in the back of your mind. Many of you are thinking that a few weeks are enough time to start to see improvements with treatments such as exercise or physical therapy. Sadly thats not going to happen. It is not unusual for it to take 3-4 months before you start to see significant improvement in your pain. Furthermore, it is not uncommon for it to take 8-12 months for a complete resolution of your symptoms. This is important I wouldnt want to seek a surgeons consultation because 4-6 weeks of therapy and exercise left you with persistent pain.

Why Do I Have Pain In The Front Of The Knee

What causes pain at the front of the knee?

The most common cause of anterior knee pain is often felt to come from a muscular imbalance or a particular pattern of weakness. This will be the case for the majority of you reading this. At least thats what we think is the most common cause. We have come a long way in evaluating runners and other people who present with pain in the front of their knees. They often show the same findings when we use high-speed cameras to videotape them while running or walking. Now, this might be a chicken vs. the egg thing. Right? Which came first, the weakness pattern or the pain? We like to think it was the weakness that came first. We are still working on proving that.

If you are a runner or cyclist, etc then your training may influence your anterior knee pain. If you train too hard, too fast, and too often then you are at a higher risk of developing anterior knee pain. Most amateur runners run too fast on their easy days and too slow on their hard days. Proper base building, for strength, endurance and conditioning our joints to adapt to distance is of paramount importance. Zone 2 running programs are extremely important even for elite runners. Try to keep your training at a continuously progressive pace. You should be slowly increasing the load, distance, or speed over time. Try not to increase your load, eg. distance, speed, etc more than 10% per week. Most runners overuse injuries are training errors.

Symptoms And Signs Of Teenage Knee Pain

The location of the pain may vary depending on the underlying cause. Some children may have anterior knee pain, while others may have posterior knee pain. The following signs and symptoms can be associated with knee pain in most teens .

Housemaids Knee Due To Other Causes

Most episodes of housemaids knee will settle with supportive treatment and do not require medicines or surgery.

Supportive treatmentThe following may be suggested:

  • Resting the knee.
  • The use of ice packs on the knee .
  • For kneeling, the use of a thick foam cushion or knee pads these can help to prevent the condition from coming back.
  • A physiotherapist can help by teaching you some exercises if your knee joint is affected by a reduced range of movement.
  • A stick or a cane can help with walking.

Treatment using medicinesNon-steroidal anti-inflammatory drugs , such as ibuprofen, can be taken for mild-to-moderate pain caused by housemaids knee. These also help to reduce inflammation.

If housemaids knee becomes more troublesome, your doctor may suggest that they draw out the fluid from the bursa, using a small needle. They can then inject a steroid medicine into the bursa to counteract the inflammation. The idea is that the fluid will no longer accumulate. Infection must be excluded beforehand, as the steroid injection can make any pre-existing infection worse. You are unlikely to develop complications after this procedure. However, occasionally the following may occur:

  • The inflammation of the bursa can come back.
  • You can develop infection, bleeding or damage to the tendon joined to the kneecap.

Surgical treatmentIn rare cases when housemaids knee does not go away, or keeps coming back and is causing intolerable symptoms, surgery may be necessary.

Before you read this, it is worth repeating the majority of you will not have the issues that I outline below. The majority of you will have pain due to a gait abnormality or muscle imbalance. We all feel the need to have a specific label for why our knee hurts. Anterior knee pain due to muscle imbalance just doesnt cut if for some of you you want something more specific. I understand that.

The point I am trying to make is, if you assign the cause of your pain to a specific structural finding or something that your MRI report mentioned, then you may have difficulty getting past that and buying into the PT or exercise strategies that have been shown to work.

  • Chondromalacia: Chondromalacia is likely a very early sign of arthritis. It involves the softening of the cartilage under your patella . As the cartilage softens, it is thought that it irritates other tissue in the knee with certain activities. Im not convinced that this is a common cause of anterior knee pain. Chondromalacia is very common, even in people with no pain.

We find chondromalacia in so many people at the time of surgery for other things, such as an ACL tear. And those people rarely have pain in the front of their knee. So it is not entirely clear why some people might have pain due to chondromalacia, and others do not. Surgery is rarely needed.
